The Analysis of Definition and Types for Medication Error Used in Foreign Countries

Abstract
Objectives: To analyze the current status of definition and types of medication error used in several countries.

Methods: Systematic search and analyses were performed to investigate the definition and types of medication error used in patient safety organizations in several countries including the United States, Europe, Australia, New Zealand, and Korea.

Results: Twenty three organizations suggested a definition or a type of medication error. The most widely used definition for medication error was that of the National Coordinating Council for Medication Error Reporting and Prevention (NCC MERP). Types of medication error highly varied by organization. Types were classified by severity criteria, process criteria, activity criteria, and responsible person criteria. Severity and activity criteria were used most frequently. The Ministry of Health & Welfare in Korea uses the definition of the NCC MERP, and the criteria suggested by the American Society of Health-System Pharmacists for the types of medication error.

Conclusion: Definitions of medication error are generally similar in most patient safety organizations, whereas types of medication error varied by organization. In part, this variation results from classification criteria for medication error.
